Renacidin packaging - help

I just received my mail order Renacidin. The packaging is a bit confusing. I expected a twist off top like a bottled water or sterile saline. It looks exactly like a medication vial where you would stick the hypodermic needle in and draw to fill the syringe. Am I supposed to put the tip of the plastic syringe thru the rubber and fill as if it was a regular shot or am I supposed to pop the whole top off? I really haven't looked at it very closely to even see if either scenario is possible. Anyone else out there that uses this have it packaged differently?
